Creating and Managing Service Request Forms

Overview

The Forms feature allows you to create and send customizable forms to vendors, service providers, and property members. Forms can be used to collect checklists, required photos, notes, and other information, making it easy to document completed work and standardize recurring processes.
Forms can also be scheduled to send automatically and include location verification when submissions are completed.



Using Form Templates

PadPulse includes several default form templates that can be used immediately or customized to fit your workflow.
Templates can be organized by category based on the intended recipient, such as:

  • Vendors
  • Property members
  • Service providers

You can also create your own templates from scratch.



Create a Custom Form

To create a new form:

  1. Navigate to Forms.
  2. Select Create New Template.
  3. Enter a name for the form.
  4. Choose a form category.
  5. Add one or more sections.

Within each section, you can include:

  • Checklist items
  • Required or optional photos
  • Notes fields

For example, a landscaping form might include separate sections for the front yard and backyard, each with its own checklist and before-and-after photo requirements.



Send a Form

To send a form:

  1. Select Send Form.
  2. Choose the form template.
  3. Select the associated property.
  4. Choose a recipient.


Recipients can be:

  • A vendor from your Vendor Directory
  • A property member
  • A custom contact using a phone number


Note: Vendors will only appear in the selection list if they have already been assigned to the selected property in the Vendor Directory.
If needed, you can send a form to someone outside your Vendor Directory by entering their name and phone number manually.



Generate and Send Forms

When sending a form, you have two options:

  • Generate and Send – Sends the form directly via SMS and tracks its status.
  • Generate and Copy – Creates a shareable link that you can send through another communication method.


Using Generate and Send is the recommended option because PadPulse tracks the form throughout the process.
Tracked forms display statuses such as:

  • Sent
  • Opened
  • Completed


You can also:

  • Send reminders
  • Copy the form link
  • Open the form yourself
  • Chat with the recipient
  • Archive completed or outdated forms


Chat with Recipients

Each form includes a built-in two-way chat.
This allows you and the recipient to:

  • Ask questions
  • Provide updates
  • Clarify instructions
  • Communicate while the form is being completed


Schedule Recurring Forms

Forms can be scheduled to send automatically on a recurring basis.
To create a recurring schedule:

  1. Select a previously created template.
  2. Choose the associated property.
  3. Select the recipient.
  4. Choose how often the form should be sent.
  5. Select the appropriate start date.
  6. Save the schedule.


Recipients can include:

  • Vendors
  • Property members
  • Custom contacts


If sending to a property member, only members assigned to the selected property will be available for selection.
Once configured, PadPulse automatically sends the form by SMS on the scheduled date and time.



View Sent Forms

The Sent Forms section displays all forms that have been distributed.
You can organize sent forms by:

  • Template
  • Property

This makes it easy to review all forms associated with a specific property or template.



Review Form Submissions

Completed forms appear in the Submissions section.
For each submission, you can view:

  • Recipient
  • Form template
  • Submission date
  • Uploaded photos
  • Submission status

Selecting a submission opens the complete form, including all completed checklist items, notes, and uploaded images.



Location Verification

When a form is submitted, PadPulse records the submitter's location.
The system compares the submission location with the property's address and displays the distance between the two locations.
This helps verify that work was completed at the correct property and can help identify submissions that may require additional review.



Important Notes

  • Default templates are included and can be customized.
  • Forms can contain checklists, required photos, optional photos, and notes.
  • Vendors must be assigned to a property before they can be selected from the Vendor Directory.
  • Generate and Send is the recommended method because it tracks form status.
  • Forms support built-in two-way chat between the sender and recipient.
  • Recurring forms can be scheduled to send automatically via SMS.
  • Submission location is automatically recorded to help verify work was completed at the correct property.
